Incident Reporting

In some circumstances, an incident or accident needs to be recorded and reported to the insurer in case it results in a future claim. If you are uncertain as to whether an incident should be reported, it is a good idea to at least use the form to record the events while they are fresh in your mind. The form does not need to be comprehensively completed, but record what detail there may be.
